Nicholas Brendon, the troubled actor best known for playing lovable sidekick Xander Harris on the hit TV series Buffy the Vampire Slayer, died from severe heart disease after suffering alarming health issues in the days leading up to his death, according to newly released reports. He was 54.
The actor’s official cause of death was revealed by the Putnam County Coroner’s Office, which confirmed Brendon died of atherosclerotic and hypertensive cardiovascular disease — a deadly condition linked to blocked arteries and high blood pressure. Investigators also found he had acute pneumonia and evidence of a previous heart attack.
Even more disturbing, the report revealed one of Brendon’s coronary arteries was reportedly 90% blocked at the time of his death.
Authorities said there were no signs of foul play at the scene after law enforcement arrived at the actor’s home following a frantic 911 call made by longtime friend Theresa Fortier.
According to the coroner, Fortier told investigators that Brendon had been battling a persistent cough and chest pain in the days before his death, but repeatedly refused medical treatment.
She reportedly urged the former TV star to go to the hospital, but he declined — allegedly worried about complications connected to recent back surgery. The report also noted Brendon had been a longtime smoker and previously suffered a heart attack years earlier but chose not to pursue additional treatment afterward.
The autopsy painted an even bleaker picture of the actor’s declining health. Medical examiners reportedly found Brendon had a severely enlarged heart, dangerously narrowed arteries, pneumonia, and inflammation in his small bowel caused by reduced blood flow linked to cardiogenic shock.
In haunting videos filmed shortly before his death, the actor could reportedly be heard coughing heavily and struggling with a raspy voice — signs investigators later said reflected his worsening condition.
Brendon’s family announced his death on March 20 in an emotional statement shared on Instagram.
“We are heartbroken to share the passing of our brother and son, Nicholas Brendon,” the family wrote. “He passed in his sleep of natural causes.”
While millions knew him as wisecracking fan favorite Xander Harris, loved by viewers alongside stars like Sarah Michelle Gellar, Alyson Hannigan and David Boreanaz, those closest to Brendon said he had recently found comfort in painting and art during his difficult final years.
The actor openly battled health and legal troubles for years. In 2022, he suffered a serious cardiac incident involving tachycardia and arrhythmia, according to a statement shared on social media at the time. Just one year earlier, representatives for Brendon revealed he was dealing with “deep-seated medical problems and immense pain” following felony prescription fraud charges.
And heartbreakingly, the months before his death were filled with mounting legal drama. Brendon was reportedly facing a lawsuit tied to unfinished home renovations and was accused during a court hearing of mistreating workers and secretly placing an AirTag on a contractor just days before he died.
Brendon rose to fame during the late ’90s and early 2000s thanks to his breakout role on Buffy the Vampire Slayer, which aired from 1997 to 2003 and became one of the most beloved cult TV shows of its era. He later appeared in films including Coherence and Unholy.
Now, fans are mourning the tragic end of a star whose life off-screen became just as dramatic — and heartbreaking — as the roles he played on television.
