The heartbreaking final chapter in the life of Peter Falk’s daughter, Jacqueline “Jackie” Falk, has been revealed after years of painful questions surrounding the family of the beloved Columbo star.

According to an autopsy report, Jackie died by suicide at her Los Angeles home in April. She was 60 years old.

Her body was discovered on April 27 after a close friend contacted police and requested a welfare check, saying she had not heard from Jackie since April 20. Authorities identified her at the scene through her driver’s license.

The medical examiner estimated Jackie had died around April 22. Toxicology testing found no alcohol or drugs in her system at the time of her death.

The report also noted that multiple notes were found in the garage, though their contents have not been publicly released.

Jackie was one of two daughters adopted by Peter Falk and his first wife, Alyce Mayo. The couple, who were college sweethearts, divorced in 1976 after 16 years of marriage. The actor later married actress Shera Danese in 1977.

In the years before Peter’s death, Jackie and her sister, Catherine Falk-Rothchild, became entangled in a bitter family dispute over access to their father as his health declined.

The Emmy-winning actor, best known to generations of fans as the rumpled but brilliant detective Columbo, had been diagnosed with Alzheimer’s disease. His daughters later claimed they were kept away from him during his final years.

In 2008, Catherine sued Danese over visitation rights and control of Falk’s estimated $7 million estate. A court ultimately appointed Danese as his custodian while allowing Catherine and Jackie limited, monitored visits.

The family pain did not end there. After Peter Falk died in 2011 at age 83, his daughters claimed they were not notified of his passing. They were also reportedly left out of his will.

Jackie always insisted the fight was never about money.

“I adored him, he adored me,” she once said.

Sources close to the family said Jackie was deeply wounded by the separation from her father and struggled with depression in the years that followed. Her death now marks another devastating turn in a family story already shadowed by illness, legal battles and long-running heartbreak.

Peter Falk remains one of television’s most enduring icons, remembered for his unmistakable voice, wrinkled trench coat and unforgettable portrayal of Lieutenant Columbo.
